Job Summary

Norfolk Community Health and Care NHS Trust is seeking a skilled and compassionate Virtual Ward Triage Clinician to join our team. As a key member of our Virtual Ward Hub, you will provide clinical triage and ongoing management of referrals, ensuring patients receive the best possible care.

Main Responsibilities
  • Assess and prioritize incoming referrals, determining the level of care required and processing referrals accordingly.
  • Contribute to the delivery of high-quality patient care by overseeing and responding to remote monitoring systems, supported by the senior clinical team.
  • Communicate regularly with patients and their carers, triaging clinical concerns and presentations as they arise, and liaising with community clinicians assigned to the Virtual Ward.
  • Develop knowledge and skills in SystmOne ledger management, identifying areas of escalation that could impact capacity.
  • Participate in multidisciplinary meetings and contribute to the requisitioning of supplies and equipment through electronic ordering systems.

Requirements
  • Registered Adult Nurse Qualification
  • Diploma or 1st-level degree
  • Current NMC registration
  • Evidence of continuing professional development
  • Experience working within a multidisciplinary team and delivering care plans to meet patient needs
