About the Role

We are seeking a highly motivated and experienced Senior Triage Clinician to join our diverse and supportive team at Norfolk Community Health and Care NHS Trust. As a key member of the Triage Hub, you will play a pivotal role in coordinating patient visits across the region and providing guidance to Triage nurses.

Main Responsibilities
  • Provide expert advice and support to other members of the primary healthcare team.
  • Offer mentorship and guidance to HUB staff, promoting a culture of excellence and best practice.
  • Be involved in the provision of relevant professional advice and expertise in matters regarding the service, ensuring seamless coordination and delivery.
  • Have oversight of the day-to-day coordination of patient visits, ensuring timely and effective care.
  • Maintain accurate, confidential, and up-to-date records for all patient contacts, adhering to strict data protection protocols.
  • Ensure activity data and clinical information is recorded in the patient-held record and on our electronic health record system.
  • Be familiar with and comply with all relevant measures under the data protection act and Caldecott guardian initiative, vulnerable adult policies, and our clinical governance framework.
  • Attend regular supervision meetings with a nominated supervisor and provide supervision for other team members, promoting a culture of continuous learning and improvement.
  • Participate in continuing professional development activity and all mandatory training as required, ensuring up-to-date knowledge and skills.
  • Practice within our professional codes of conduct and protocols, upholding the highest standards of patient care and safety.
  • Promote the effective use of local services by closer networking and collaboration.

Person Specification
  • Evidence of working at a relevant level within the community setting, with a strong track record of patient care and coordination.
  • Knowledge of additional specialist areas through in-house training and short courses, with a focus on patient-centered care.
  • Registered general nurse with a strong commitment to patient safety and well-being.
  • Assessor/mentorship preparation; ENB 998 or equivalent, with a focus on developing the skills of others.
  • Current NMC registration, with a strong commitment to ongoing professional development.
  • Evidence of continuing professional development, with a focus on staying up-to-date with the latest best practices and research.
Personal and People Development
  • Be reliable and dependable, with a strong commitment to patient care and safety.
  • Professional appearance and demeanor, with a focus on promoting a positive and respectful work environment.
  • Be able to kneel, bend, and stoop, and work in cramped environments, with a focus on patient mobility and accessibility.
  • Be able to manoeuvre limbs of around 5-6 kg, with a focus on patient care and safety.
  • Be able to manoeuvre patients using handling aids, with a focus on patient dignity and respect.
Other
  • The post holder will be required to work shifts over a seven-day period, with a focus on providing flexible and responsive care.
  • The post holder will need to be passed fit to perform full duties of the post through occupational health, with any reasonable aids and adaptations if necessary.
  • Must hold a full and valid driving license and have access to a vehicle, with a focus on providing timely and effective care.
  • Able to communicate effectively in written and verbal English language, with a focus on patient safety and well-being.
