Job Summary

Norfolk Community Health and Care NHS Trust is seeking a skilled Virtual Ward Triage Clinician to join our team. As a key member of our Virtual Ward Hub, you will provide clinical triage and support to patients referred to our service.

Main Responsibilities
  • Assess and prioritize patient referrals, determining the level of care required and processing referrals accordingly.
  • Contribute to the delivery of high-quality patient care by overseeing and responding to remote monitoring systems, supported by the senior clinical team.
  • Communicate regularly with patients and their carers, triaging clinical concerns and presentations as they arise, and liaising with community clinicians assigned to Virtual Ward across the sector.
  • Develop knowledge and skills in SystmOne ledger management to identify areas of escalation that could affect capacity.
  • Participate in multi-disciplinary/multi-agency meetings as appropriate.

Professional Requirements
  • Registered Adult Nurse Qualification
  • Diploma or 1st level degree
  • Current NMC registration
  • Evidence of continuing professional development
Desirable Skills and Experience
  • Experience of working within a multidisciplinary team
  • Experience of working with patients with multiple pathologies and delivering care plans to meet their needs
  • Knowledge of SystmOne
Working Arrangements

This is a 7-day service, and some weekend working will be expected. Hours of service delivery are 8am-6:30pm.

The post holder will need to be passed fit to perform full duties of the post through occupational health with any reasonable aids and adaptations if necessary.
