Job Overview

We are seeking a qualified Health Visitor to deliver early intervention and support through the Healthy Child Programme in City and Hackney, London.

Main Responsibilities:
  • To provide personalised care to children and families, ensuring optimal outcomes.
  • To manage vulnerable children by attending multi-disciplinary team meetings and collaborating with local authorities.
  • To develop and implement health promotion activities, including integrated development reviews using ASQ and ELIM questionnaires.
  • To supervise the nursery nurse team's health promotion work, working closely with local children's centres and parents to provide tailored advice that supports children's development.
