Band 6 Specialist Nurse Practitioner Role

We are seeking a highly skilled and experienced Clinical Nurse Specialist to join our Hull and East Riding Community Specialist Palliative Care Team.
The successful candidate will be employed in a band 6 development role and will become a band 7 once the essential qualifications for the role are achieved.
Key Responsibilities- Deliver specialist, high-quality care to patients with advanced, progressive illness and their families and carers.
- Combine clinical leadership, expertise, and complex palliative and end-of-life care management to promote palliative and end-of-life care.
- Work within a multi-disciplinary team to deliver high-quality patient care.
- Develop and maintain advanced palliative care clinical experience, knowledge, and skills.
- Undertake Non-medical Prescribing and other specified essential qualifications within a specific timeframe.
- Qualified to degree level.
- Minimum of 2 years experience working within a palliative care setting.
- Advanced palliative care clinical experience, knowledge, and skills.
- Commitment to the delivery of high-quality patient care.
