Job Title: Band 6 Specialist Nurse Practitioner

We are seeking a highly skilled and experienced Band 6 Specialist Nurse Practitioner to join our Hull and East Riding Community Specialist Palliative Care Team.

Main Responsibilities:
  • Provide high-quality, specialist palliative care to patients with advanced, progressive illnesses and their families and carers.
  • Develop and implement complex care plans, working closely with multidisciplinary teams to ensure seamless care delivery.
  • Conduct advanced clinical assessments, making informed decisions to promote palliative and end-of-life care.
  • Lead and develop clinical practice, promoting evidence-based care and best practices within the team.
  • Collaborate with healthcare professionals to design and deliver teaching and training programs, enhancing the skills of colleagues.
Requirements:
  • Registered General Nurse with a minimum of 2 years' experience in palliative care.
  • Advanced palliative care clinical experience, knowledge, and skills.
  • Commitment to delivering high-quality patient care and a proactive approach to team working and change management.
  • Leadership, negotiation, and facilitation skills, with the ability to develop effective working relationships with a multi-professional team.
  • Excellent communication and organizational skills, with the ability to manage competing priorities.
