Band 6 Specialist Nurse Practitioner

We are seeking a highly skilled and experienced Clinical Nurse Specialist to join our team in Hull and East Riding. As a key member of our multi-disciplinary team, you will play a vital role in delivering specialist, high-quality care to patients with advanced, progressive illness and their families and carers.
The successful candidate will be employed in a band 6 development role and will become a band 7 once the essential qualifications for the role are achieved. This community-based post combines clinical leadership, expertise, and complex palliative and end-of-life care management to promote palliative and end-of-life care.
Key Responsibilities:
- Deliver high-quality patient care in a palliative care setting
- Provide clinical leadership and expertise in palliative care
- Manage complex palliative and end-of-life care cases
- Develop and implement palliative care plans
- Collaborate with multidisciplinary teams to deliver care
Requirements:
- Registered Nurse with a minimum of 2 years experience in palliative care
- Advanced palliative care clinical experience, knowledge, and skills
- Commitment to delivering high-quality patient care
- Qualification to degree level
- Willingness to undertake Non-medical Prescribing and other specified essential qualifications
