About the Role

We are seeking a highly skilled and experienced Band 6 Nurse Practitioner to join our Community Learning Disability Team. As a key member of our team, you will be responsible for providing high-quality nursing care to adults with learning disabilities, working closely with other healthcare professionals to ensure comprehensive support.

Main Responsibilities
  • Conduct initial contact assessments and learning disability screening assessments for individuals entering our service.
  • Develop and implement care plans and risk assessments for individuals, working collaboratively with other healthcare professionals.
  • Supervise and support junior members of the team, providing guidance and mentorship to ensure high-quality care.
  • Assume responsibility for the team in the absence of the Band 7 Nursing Lead, ensuring seamless continuity of care.

Person Specification

We are looking for a highly skilled and experienced nurse with a strong background in learning disabilities. Essential qualifications and knowledge include:

  • Part III or 13 of the NMC live register.
  • A full understanding of relevant policy, legislation, drivers, and their application to clinical and service areas.

Desirable skills and competencies include:

  • Demonstrable experience of working in the specific field of learning disabilities.
  • Ability to lead and motivate staff to embrace change.
