About the Role

We are seeking a highly skilled and experienced Band 6 Nurse Practitioner to join our Community Learning Disability Team. As a key member of our team, you will be responsible for providing high-quality care to individuals with learning disabilities, working closely with other professionals to ensure comprehensive support.

Main Responsibilities
  • Conduct initial contact assessments and learning disability screening assessments for new service users.
  • Develop and implement care plans and risk assessments in collaboration with the individual's care team.
  • Supervise and support junior staff members, ensuring they receive the necessary guidance and mentorship.
  • Assume responsibility for the team in the absence of the Band 7 Nursing Lead.
Person Specification

We are looking for a candidate with:

  • Part III or 13 of the NMC live register.
  • A full understanding of relevant policy, legislation, drivers, and their application to clinical and service areas.
  • Evidence of continuing professional development (CPD) relevant to the clinical area at specialist level of care.
  • Evidence of development of specialist practice nursing skills and the ability to demonstrate the impact on practice change/development.
