We are seeking a highly skilled and experienced Band 6 Nurse Practitioner to join our Community Learning Disability Team. As a key member of our team, you will be responsible for providing high-quality care to individuals with learning disabilities, working closely with other professionals to ensure comprehensive support.
Main Responsibilities- Conduct initial contact assessments and learning disability screening assessments for new service users.
- Develop and implement care plans and risk assessments in collaboration with the individual's care team.
- Supervise and support junior staff members, providing guidance and mentorship as needed.
- Assume responsibility for the team in the absence of the Band 7 Nursing Lead.
- Part III or 13 of the NMC live register.
- Full understanding of relevant policy, legislation, and drivers.
- Evidence of continuing professional development (CPD) relevant to the clinical area.
- Demonstrable experience of working in the specific field.
