Devon Adult Transitions Specialist

Proud to Care Devon seeks a compassionate and experienced Adult Transitions Specialist to join our services in Devon. This role is essential in supporting individuals with mental health needs as they transition from children's to adult services.
The successful candidate will work directly with young people, families, and a multidisciplinary team to ensure a smooth, person-centred transition experience that promotes independence, dignity, and holistic well-being.
About the Role:- This is a full-time social care professional opportunity within our Adults Services team, aligning with the national Professional Capability Framework (PCF) for Social Workers.
- You will work closely with a multidisciplinary team, comprising of two Team Managers, Social Workers, Social Care Assessors, Occupational Therapists, and Business/Information Support Officers.
- We are looking for an experienced and qualified Social Worker who can demonstrate a commitment to ongoing professional development.
Key Responsibilities:
- Work with young people, families, and a multidisciplinary team to ensure a smooth transition experience.
- Develop strong links with Children's Services and Education to improve outcomes for young people.
- Implement Care Act principles to engage with young people with mental health needs.
