Job Description

We are seeking a dedicated and experienced Mental Health Adult Transitions Social Worker to join our services in Devon.

About the Role

This role is essential in supporting individuals with mental health needs as they transition from children's to adult services. You will work directly with young people, families, and a multidisciplinary team to ensure a smooth, person-centred transition experience that promotes independence, dignity, and holistic well-being.

Duties and Responsibilities
  • Liaise with CAMHS, childrens services, and support with promoting a smooth and planned transition for young people presenting with mental health as their primary need or with dual diagnosis.
  • Work closely with the TASC team in Exeter and receive practice support from DPT Social Care.
  • Promote independent living skills and engage with young people to facilitate their ability to engage with their community, gain employment, and live and enjoy their adult life as independently as possible.
Requirements
  • An experienced and qualified Social Worker (CQSW, DipSW, BA Hons, MA or equivalent) who is registered to practice with Social Work England.
  • A comprehensive working knowledge of the issues currently impacting on social care delivery and up-to-date with policy and practice development.
  • A current full driving licence and access to a vehicle due to the requirement to travel significant distances as the role is Devon Wide.

  • A competitive salary of £32,000-£43,000 per annum, depending on experience.
