Job Summary

This is an exciting opportunity for a compassionate and experienced Mental Health Social Worker to join our Transition in Adult Social Care (TASC) Team and Devon Partnership Trust (DPT) Adult Social Care. As a key member of our team, you will play a vital role in supporting individuals with mental health needs as they transition from children's to adult services.

Key Responsibilities
  • You will work directly with young people, families, and a multidisciplinary team to ensure a smooth, person-centred transition experience that promotes independence, dignity, and holistic well-being.
  • Liaise with CAMHS, children's services, and support the promotion of a smooth and planned transition for young people presenting with mental health as their primary need or with dual diagnosis.
  • Collaborate with TASC, Childrens Services, Education, and voluntary agencies to improve outcomes for young people and those who care for them as they transition into adulthood.
Requirements
  • A relevant professional social work qualification recognised by the regulatory body, Social Work England.
  • Satisfactory completion of the Assessed & Supported Year in Employment (ASYE).
  • Registration with the regulatory body, Social Work England.
What We Offer
  • A competitive salary of £40,000 - £55,000 per annum, dependent on experience.

Location

This role is based in Exeter, Devon, and requires travel throughout the county due to the requirement to visit young people and families in their homes and communities.
