Job SummaryThis is an exciting opportunity for a compassionate and experienced Mental Health Social Worker to join our Transition in Adult Social Care (TASC) Team and Devon Partnership Trust (DPT) Adult Social Care. As a key member of our team, you will play a vital role in supporting individuals with mental health needs as they transition from children's to adult services.
Key Responsibilities- You will work directly with young people, families, and a multidisciplinary team to ensure a smooth, person-centred transition experience that promotes independence, dignity, and holistic well-being.
- Liaise with CAMHS, children's services, and support the promotion of a smooth and planned transition for young people presenting with mental health as their primary need or with dual diagnosis.
- Collaborate with TASC, Childrens Services, Education, and voluntary agencies to improve outcomes for young people and those who care for them as they transition into adulthood.
- A relevant professional social work qualification recognised by the regulatory body, Social Work England.
- Satisfactory completion of the Assessed & Supported Year in Employment (ASYE).
- Registration with the regulatory body, Social Work England.
- A competitive salary of £40,000 - £55,000 per annum, dependent on experience.
