Diabetes Care Specialist

At Grandview Medical Centre Family Health Team, we are seeking a dedicated RN or RPN to fill the role of a Diabetes Educator. This is a permanent, full-time position that requires strong clinical expertise and excellent communication skills.
Salary Range:$85,000 - $110,000 per year, depending on experience.
About the Role
This Diabetes Educator will work as part of our multidisciplinary team to provide support, guidance, and education to patients with diabetes. Key responsibilities include:
- Providing care and education to patients with diabetes, applying best-practice standards.
- Conducting in-depth assessments to establish and identify priorities for patient care.
- Developing and delivering education tailored to individual learning styles and patient readiness.
Requirements
To be successful in this role, you will need:
- A minimum of 3 years' experience working in diabetes care and management.
- Certified Diabetes Educator preferred.
- Strong understanding of current clinical practice guidelines related to diabetes.
