Job Summary
The Grandview Medical Centre Family Health Team is seeking an experienced RN or RPN to join our team as a Diabetes Educator. This is a critical role that requires strong clinical expertise and excellent communication skills.

Key Responsibilities
This position involves providing care and education to patients with diabetes, conducting assessments, developing care plans, and collaborating with other healthcare professionals. Other key responsibilities include:
  • Triage referrals received by the Family Health Team.
  • Conduct follow-up appointments with patients.
  • Ensure quality and integrity of patient information through accurate use of electronic medical records.

Education and Qualifications
Applicants must have a minimum of 3 years' experience working in diabetes care and management. Certified Diabetes Educator status is preferred. Strong understanding of current clinical practice guidelines related to diabetes is essential.

What We Offer
We offer a competitive salary range of $85,000 - $110,000 per year, depending on experience, as well as a comprehensive benefits package and access to continuing education opportunities.
