Key Responsibilities:

  • Manage complex patient cases, developing individualized care plans and working with multidisciplinary teams to ensure seamless care delivery.
  • Deliver DESMOND Structured Education to newly diagnosed patients, empowering them to take control of their diabetes management.
  • Collaborate with GPs and other healthcare professionals to promote best practices and improve health outcomes.

Salary: £48,000 - £68,000 per annum pro rata, depending on experience.
