Diabetes Nurse Practitioner

This role offers a competitive annual salary of £42,000 - £57,000 and is ideal for a motivated and experienced Diabetes Specialist Nurse looking to further develop their skills and expertise.
About the RoleThis exciting opportunity has arisen for a highly skilled Diabetes Specialist Nurse to join our team in Cambridge. As a key member of the multidisciplinary team, you will be responsible for managing a caseload of patients with complex diabetes needs, providing expert advice and education to healthcare professionals, and promoting better understanding of diabetes through education, support, and counselling.
You will work closely with patients, their families, and healthcare professionals to develop individualised care plans, using clinical reasoning skills and patient-centred goals to assess, plan, implement, and evaluate interventions. Your expertise will also include acting as a resource for general practice, working to reduce admissions, improve diabetes outcomes, and enhance quality of life for individuals with diabetes.
Key Requirements- Level 1 RN/BSc Hons Degree.
- NMC Registration.
- DESMOND Trained and Accredited or willingness to undertake training and accreditation.
- Evidence of audit and data analysis.
- Counselling Skills.
