Role Overview

This challenging and rewarding role offers a competitive annual salary of £42,000 - £57,000 and is ideal for a skilled Diabetes Specialist Nurse looking to further develop their skills and expertise. You will be responsible for managing a caseload of patients with complex diabetes needs, providing evidence-based nursing advice, and promoting better understanding of diabetes through education, support, and counselling.

You will work closely with healthcare professionals, patients, and their families to develop individualised care plans, using clinical reasoning skills and patient-centred goals to assess, plan, implement, and evaluate interventions. Your expertise will also include acting as a resource for general practice, working to reduce admissions, improve diabetes outcomes, and enhance quality of life for individuals with diabetes.

Responsibilities
  • Plan, implement, and evaluate individualised specialist patient care programmes based on evidence-based practice and clinical reasoning skills.
  • To be professionally and legally accountable for all aspects of own work, including management of patients in your care.
  • Promote better understanding of diabetes for patients, their families, and carers, and other agencies involved with the patient through education, support, and counselling.
