Advanced Practice Nurse

**Overview:**
As an Advanced Practice Nurse - Long Term Conditions, you will be responsible for managing patients with complex health needs and providing high-quality care to our patients. The ideal candidate will have experience working in a primary care environment and be able to work autonomously as well as part of a team.
Main Duties:- Support patients in making lifestyle modifications to improve their health outcomes and prevent disease progression.
- Coordinate care for patients with complex health needs, liaising with other healthcare professionals and community services, as necessary.
The post holder will actively engage in practice / patient engagement and play a vital role in providing high-quality care to our patients.
About Us:Cherry Hinton & Brookfields Medical Centre has a supportive and experienced team, offering full or part-time hours and a competitive salary scale based on experience. Our centre serves nearly 10,000 patients and offers a traditional face-to-face consulting service with the support of a bigger network in the form of Cambs GP Network.
**Requirements:**
- Registered Nurse with valid NMC registration.
- Experience of working in a primary care environment.
- Strong IT skills.
**Salary:** £45,180 - £61,610 per annum pro rata. A competitive salary scale based on experience.
