We are seeking an experienced Practice Nurse to join our committed, hard-working and multi-disciplinary team of clinicians at Cherry Hinton & Brookfields Medical Centre.

The post holder is responsible for ensuring delivery of safe and effective nursing care to the whole practice population and accountable for nursing service delivery.

">Main Duties:">
  1. ">
  2. Support patients in making lifestyle modifications to improve their health outcomes and prevent disease progression.">
  3. To identify, manage and support Patients at risk of developing long-term conditions, preventing adverse effects to patients' health.">
  4. Coordinate care for patients with complex health needs, liaising with other healthcare professionals and community services, as necessary.">
  5. Collaborate with multidisciplinary teams to develop and implement care plans that address the holistic needs of patients.">
