Job Description

Our Community Support Practitioner role involves:

  • Social Support - assisting individuals to engage with their communities through outings to leisure activities like day centres, cinema, swimming, etc.
  • Medical Support - administering and monitoring medication.
  • Practical Support - helping people maintain their tenancies by encouraging daily household tasks such as general housework, laundry, ironing, cooking, shopping.
