Role Overview: We are currently recruiting for a skilled hospital social worker to join our team in the Cotswolds area. As a key member of our team, you will be responsible for assessing the needs of patients and providing support to ensure their discharge from hospital is smooth and successful.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' needs and develop tailored plans to support their discharge.
  • Collaborate with healthcare professionals and other stakeholders to ensure effective communication and collaboration.
  • Manage safeguarding enquiries and complex cases, working closely with colleagues and external agencies as needed.

Requirements:

  • AASW registered
  • Proven experience in hospital social work
  • Strong understanding of local authority policies and procedures
  • Exceptional interpersonal and communication skills
  • Ability to work effectively in a fast-paced environment

Remuneration:

  • Hourly rate up to £35 per hour
  • Accommodation/commuter mileage allowance available for those living in the area
