Mental Health Liaison Practitioner

We are seeking a motivated and enthusiastic clinician to join our dynamic Mental Health Liaison Team based at the Stonebow Unit, Hereford. The successful candidate will work with patients who present within the acute hospital with a variety of mental health conditions alongside complex physical comorbidities. The role will involve identifying and supporting patients, completing comprehensive risk assessments, and working collaboratively with the Multi-disciplinary team to provide a fully integrated service.
Key Responsibilities
* Use their initiative and take responsibility for themselves and the quality of their work and the service they provide to patients.
* Proactively engage in formal and informal education of Acute Trust staff and other partnership agencies as well as appropriate students.
* Actively and creatively engage patients and carers encouraging them to make appropriate use of the mental health and voluntary services available as part of a process of recovery and crisis management.

Qualifications
Essential
- Registered Mental Nurse, Occupational Therapist or Social Worker
- A relevant degree or diploma
- Appropriate additional post-registration / graduation qualifications
- Evidence of on-going continuing professional development
- Evidence of on-going continuing professional development

- Ability to communicate clearly and effectively in English in both verbal and written formats.
- Computer literacy.
Essential
- At least 2 years post qualification experience in an appropriate mental health setting
- Working within and utilising a supervisory framework
- Undertaking regular multi-disciplinary assessments
- Undertaking mental state and self-harm assessments
- Assessing and managing risk
- Working within the Care Coordination framework
- Supervision of junior staff and students
- Experience of working in a Liaison or Crisis & Home Treatment service
