Job Summary

We are seeking a highly skilled and experienced Community Mental Health Nurse to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. The successful candidate will be responsible for providing high-quality care to patients with dementia and mental health conditions.

Main Responsibilities
  • Manage a caseload of patients to ensure timely assessment, diagnosis, and treatment.
  • Monitor treatments and make adjustments as necessary.
  • Develop and implement effective discharge planning and coordination.
  • Undertake physical health observations and take appropriate action when necessary.
  • Monitor, evaluate, and modify treatment to measure progress and ensure effectiveness.
  • Apply a high level of understanding of dementia and provide advice on management, healthy lifestyles, and well-being.
  • Carry out complex risk assessments.
  • Timely and accurate reporting and escalation of incidents and safeguarding concerns.
  • Attend regular multidisciplinary meetings.
  • Participate in undertaking professional cover/duty.

Job Requirements
  • RMN (Degree/Diploma and working towards degree).
  • Mentors Award.
  • Evidence of Continuing Professional Development.
  • Commitment to patient-centred, non-discriminatory practice.
  • Commitment to lifelong learning.
  • Able to meet the travel requirements of the role.
  • Documented evidence of continuing professional development.
  • Significant post-registration experience, including experience within mental health.
  • Minimum 1 year within/related specialty.
  • Extensive clinical experience, including individual and group work.
