About the Role

We are seeking a highly skilled and dedicated Community Mental Health Nurse to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients with complex mental health needs.

Key Responsibilities
  • Manage a caseload of patients with complex mental health needs, using evidence-based and patient-centered principles to assess, plan, implement, and evaluate interventions.
  • Work collaboratively with multidisciplinary teams to provide comprehensive care and support to patients and their families.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Provide leadership and supervision to junior staff, promoting a culture of excellence and continuous learning.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practice in mental health care.
Requirements
  • Registered Mental Health Nurse (RMN) with a degree or diploma in nursing and working towards a degree.
  • Mentorship Award and evidence of continuing professional development.
  • Specialist knowledge and application of interventions relevant to patient group.
  • Knowledge and experience of audit procedures, mental health legislation, and clinical governance.
  • Ability to communicate clearly and effectively in English in both verbal and written formats.
  • Ability to work autonomously and set own priorities, with a positive attitude and ability to act on feedback.
