Main Responsibilities
  • Manage a caseload of patients with complex mental health needs, providing timely and effective assessments, diagnoses, and treatments.
  • Monitor treatments and make adjustments as necessary to ensure optimal patient outcomes.
  • Develop and implement discharge plans, ensuring a smooth transition for patients back into their communities.
  • Collaborate with multidisciplinary teams to provide comprehensive care and support to patients and their families.
  • Participate in regular multidisciplinary meetings to discuss patient progress and develop care plans.
