Job Description

The primary objective of this role is to empower individuals to make effective long-term changes that halt or reverse the progression of Type 2 Diabetes. This is achieved through delivering evidence-based interventions and providing support to clients in improving their health and wellbeing.


  • Deliver group-based interventions focusing on diet, nutrition, physical activity, and behaviour change.
  • Facilitate safe and positive environments for clients to achieve their goals.
  • Promote the Healthier You NHS Diabetes Prevention Programme internally and externally.
  • Engage in ongoing training relevant to the role, such as Behaviour Change Level 1 and 2.

Requirements

We are looking for an individual who can combine a high level of health improvement knowledge with first-class communication and motivational skills. Experience of delivering group-based interventions and a real understanding of the impact personal characteristics and circumstances can have on health behaviours is essential.
