Community Health Advocate for Children

We are seeking a highly skilled Community Health Advocate for Children to join our team in Northamptonshire. As a Community Health Advocate for Children, you will play a crucial role in providing specialist multi-disciplinary healthcare assessment and treatment services locally for children who have a learning disability presenting with physical health needs, a mental illness and/or challenging behaviour.
About the JobYou will work closely with the Children's Community Team for People with a Learning Disability (CCTPLD) to develop and implement individualised care plans that meet the unique needs of each child. Your expertise will enable you to provide specialist assessment and intervention to patients with learning disabilities in the community, promoting optimal health and wellbeing outcomes.
Requirements- Recognised Nursing qualification - RNLD
- Experience of engaging and communicating effectively with people at all levels
- Ability to work autonomously and collaboratively within a team
The estimated annual salary for this role is £35,000 - £50,000 depending on experience.
