Children's Learning Disability Health Care Practitioner

We are seeking a Children's Learning Disability Health Care Practitioner to join our team in Northamptonshire. This is a fantastic opportunity for an experienced healthcare professional to work with children and young people who have learning disabilities, providing specialist assessment and intervention services.
The successful candidate will work closely with the Community Nursing Team, holding a caseload of individuals with a broad range of needs. They will provide specialist assessment and intervention to patients with learning disabilities in the community, working with various issues related to physical health and emotional wellbeing.
Responsibilities- Providing specialist assessment and intervention to patients with learning disabilities in the community
- Working with various issues related to physical health and emotional wellbeing
- Collaborating with the person, their network of care, mainstream health services, and the multidisciplinary team
- Reflecting on own practice and performance through effective use of professional and operational supervision and appraisal
