Job Title: Community Rehabilitation Specialist

Location: Royal Borough of Windsor and Maidenhead

Salary:$28,000 - $40,000 per annum

Job Description:

We are looking for a highly motivated and experienced Community Rehabilitation Specialist to join our team in the Royal Borough of Windsor and Maidenhead. As a member of our multidisciplinary team, you will work closely with Occupational Therapists, Physiotherapists, and other healthcare professionals to provide person-centered rehabilitation services to individuals in their own homes.

Main Responsibilities:

  • To deliver safe and effective rehabilitation programs to residents of RBWM in their own homes.
  • To work closely with the therapy team to develop and implement personalized care plans that meet the unique needs of each service user.
  • To provide education and training to family members and caregivers on how to support the service user's recovery and independence.
  • To maintain accurate records and reports to ensure seamless communication between healthcare professionals and stakeholders.

Person Specification:

  • Experience of working in a community-based setting.
  • Qualifications in occupational therapy, physiotherapy, or a related field.
  • Strong communication and interpersonal skills.
