Senior Staff Nurse Community Services Specialist

We are seeking a highly skilled and experienced Senior Staff Nurse to join our Community Services team. As a Senior Staff Nurse, you will play a key role in the development and delivery of our IPS service, providing specialist palliative and end-of-life care to adults with advanced life-limiting illnesses.
Key Responsibilities:
- Develop and deliver the IPS service, ensuring an excellent standard of care to patients and their families
- Lead the IPU team when on shift, in the absence of a Sister
- Provide timely and individual care based on need, using referral criteria, regular patient review, assessment, re-assessment and discharge criteria as appropriate
- Deliver direct care to patients, including symptom control and administration of medicines in line with Thames Hospice Policy
- Communicate effectively with patients, their families and professional colleagues to ensure a safe and needs-led care pathway
- Work as a member of the multidisciplinary team to ensure a coordinated and defined management plan/plan of care for each patient
Requirements:
- Registered Nurse with post-registration nursing experience
- Knowledge and understanding of current developments in palliative care
- Competent in extended skills, including IV drug administration, cannulation and male catheterization
- Awareness of national Care Quality Commission monitoring requirements
- Experience of undertaking nursing assessments and care planning for patients with complex needs
