About the Role

We are seeking a highly skilled and experienced Senior Mental Health Practitioner to join our team at Devon Partnership NHS Trust. As a key member of our community mental health team, you will be responsible for delivering high-quality mental health services to individuals with complex needs.

Key Responsibilities
  • Manage a defined caseload of clients with complex mental health presentations, providing expert assessment and treatment.
  • Work collaboratively with multi-disciplinary teams to ensure consistent and well-coordinated care.
  • Liaise with a wide range of statutory and non-statutory agencies to ensure seamless service delivery.
  • Contribute to the clinical leadership of the team, providing supervision and participating in team and service development.
  • Develop and deliver care packages that are personalized and evaluated, taking into account the physical health and social needs of the individual.
Requirements
  • A professional mental health qualification and registration (RMN, BSc/Diploma in OT, SROT, Social Work Degree or equivalent, Community Nursing Degree).
  • Substantial experience of managing an extensive and complex caseload of clients with a range of complex mental health presentations.
  • Working within a community mental health setting in either the statutory or voluntary sector.
  • Providing clinical supervision.
