We are seeking a skilled and compassionate Mental Health Practitioner to join our team at Devon Partnership NHS Trust. As a key member of our community mental health team, you will play a vital role in providing high-quality care and support to individuals experiencing mental health issues.
Key Responsibilities- Conduct comprehensive assessments and develop personalized care plans to address the complex needs of service users.
- Engage with service users and their families/carers in a non-judgmental and empathetic manner, promoting assertive community treatment and preventing hospital admissions.
- Develop and maintain therapeutic relationships to facilitate recovery and reduce the risk of relapse.
- Collaborate with multidisciplinary teams to provide proactive risk management and ensure the basic needs of service users are met.
- Provide interventions and support to service users experiencing co-morbid problems with substance use/misuse.
- Engage service users and their families in relapse prevention and crisis planning, promoting autonomy and independence.
- Relevant professional mental health qualification and current registration with no restrictions.
- Evidence of ongoing professional development and training in relevant areas, such as safeguarding and mental health practice.
- Substantial experience working within mental health settings, managing caseloads and providing clinical supervision.
- Excellent communication and interpersonal skills, with the ability to engage with service users and their families in a compassionate and non-judgmental manner.
