Main Duties
Provide initial and ongoing assessment of nursing needs of patients and carers in the community, taking into account physical, social, and emotional factors.
Work as an autonomous practitioner in planning and implementing nursing treatment programmes.
Advise and instruct carers and patients on health and wellbeing issues, and self-care strategies.
Carry out specialist and shared complex nursing assessments, including continuing healthcare, medication review, prescribing, and palliative care.
Utilise a broad range of specialised nursing skills and knowledge, aligning practice to the organisation's Medicines Policy for Registered Professionals and the NMC Standards for Medicines Management.
About Us
The Newton Abbot Intermediate Care Team and Community Nursing Team are based at Sherborne House. The Community Nurse Team is also based at Tower House Surgery in Chudleigh. We cover a large geographical patch covering the practice population of nine GP surgeries.
We are a busy but friendly and supportive team committed to staff development through education and training. We help staff realise their potential in topics of interest to them and encourage career pathways in Community Nursing. We pride ourselves on delivering high standards of care and building relationships within the communities we serve.
Person Specification
Qualified Registered Nurse Level 1
BSc (Hons) in Community Health Nursing (District) or equivalent, or working towards
Ability to provide detailed advice to other specialists and non-specialists
Evidence of continual professional development
Ability to work as an autonomous practitioner
Proven relevant knowledge and experience
Specific Skills
Holistic client assessment skills using an identified model of care
Evidence of a range of clinical skills and ability to undertake training where required
Ability to make sound clinical judgements and decisions in response to complex or crisis situations
