Key Responsibilities- Assessment and Treatment: Conduct comprehensive assessments and develop personalized treatment plans to address the mental health needs of our service users.
- Case Management: Manage a caseload of clients with a range of mental health presentations, providing ongoing support and guidance to ensure optimal outcomes.
- Collaboration and Communication: Work effectively with multidisciplinary teams, including primary care teams, external agencies, and service user groups, to ensure seamless care coordination and information sharing.
- Risk Assessment and Management: Identify and mitigate risks associated with mental health presentations, ensuring the safety and well-being of service users and staff.
- Service Development: Contribute to the development and implementation of mental health services, including the design and delivery of care packages and the evaluation of service effectiveness.
- Supervision and Training: Provide clinical supervision and training to junior colleagues, promoting professional development and ensuring high standards of practice.
- Quality Improvement: Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that promote excellent care and service delivery.
- Qualifications: Possess an appropriate professional mental health qualification and registration, such as RMN, OT, or Social Worker.
- Experience: Have experience of working with older adults with mental health problems, managing a caseload of clients with a range of mental health presentations, and providing clinical supervision.
- Skills: Demonstrate excellent communication and interpersonal skills, with the ability to engage with clients experiencing a wide range of mental health presentations. Possess strong analytical and problem-solving skills, with the ability to assess risk and develop effective solutions.
