Job Title: Adults Mental Health Senior Practitioner

We are seeking a highly skilled and experienced Senior Mental Health Practitioner to join our team at Devon Partnership NHS Trust. As a key member of our community mental health team, you will play a vital role in delivering high-quality care and support to individuals experiencing mental health issues.

About the Role

As a Senior Mental Health Practitioner, you will be responsible for assessing, planning, and implementing care packages that meet the unique needs of each individual.

You will work closely with a multidisciplinary team, including nurses, therapists, and other healthcare professionals, to provide comprehensive care and support. Your expertise and experience will be invaluable in helping us to deliver our vision of providing high-quality, patient-centered care.

Key Responsibilities
  • Assess and develop care plans that meet the needs of individuals with complex mental health needs
  • Work collaboratively with the multidisciplinary team to deliver high-quality care and support
  • Provide clinical supervision and guidance to junior staff members
  • Contribute to the development and implementation of new care pathways and initiatives
  • Participate in quality improvement activities and contribute to the continuous development of our services
Person Specification

We are looking for an experienced and skilled Senior Mental Health Practitioner who possesses the following qualities:

  • Professional mental health qualification (e.g., RMN, CQSW/ASW, BSc/Diploma in OT)
  • Current registration with a professional body (e.g., NMC)
  • Substantial experience of managing a busy caseload of individuals with complex mental health needs
  • Experience of working in a multidisciplinary team environment
  • Excellent communication and interpersonal skills
