About the Role

We are seeking a highly skilled Clinical PCN Pharmacist to join our team in City and Hackney. As a key member of our primary care network, you will work closely with general practitioners, community pharmacists, and other healthcare professionals to deliver high-quality patient care.

As a Clinical PCN Pharmacist, you will be responsible for developing and implementing medicines optimisation strategies to improve patient outcomes and reduce waste. You will also provide specialist expertise in the use of medicines, support patients with long-term conditions, and contribute to the Quality and Outcomes Framework (QOF) targets.

The successful candidate will have a strong background in pharmacy, with a minimum of 2 years' post-graduate experience. They will be registered as a pharmacist with the General Pharmaceutical Council and possess excellent communication and leadership skills.

Salary: £50,000 - £70,000 per annum
