Pharmacy Medicines Optimisation Professional Opportunity

We are looking for a skilled and dedicated Pharmacy Medicines Optimisation Professional to join our team at Central & North West London NHS Foundation Trust.

About the Position:

The post offers an excellent opportunity to develop your clinical and professional knowledge and skills in both rehabilitation and elderly care healthcare services, working closely with the multi-disciplinary team in an inpatient setting.

Responsibilities:
  • Identify opportunities to engage with patients/carers, confidently complete medicine reconciliation, and deliver medication counselling.
  • Organise your workload and uphold standards of pharmacy practice, referring and reporting concerns when appropriate.
  • Maintain general medicines management skills, including stock management, ordering patient medications, reducing wastage, and maintaining secure environments.
Requirements:
  • GPhC recognised combined Pharmacy Technician qualification (NVQ Level 3 or equivalent)
  • Current registration with GPhC as a Pharmacy Technician
  • Experience of supporting or developing technician services, liaison within a multidisciplinary team
Additional Information:
  • The successful candidate will participate in the weekend and bank holiday rota, following a period of induction and training.
  • Salary: £38,890 - £44,503 per annum
