We are seeking a skilled and compassionate Care Coordinator to join our team at Turning Point. As a Care Coordinator, you will play a vital role in supporting individuals with learning disabilities across England.
The ideal candidate will be passionate about providing exceptional care and support, with a strong focus on promoting independence and enhancing daily living skills. You will work closely with our residents, their families, and our multi-disciplinary teams to develop personalized care plans that meet the unique needs of each individual.

About the Role:
- Coordinate care activities and develop personalized care plans

Requirements:
- Degree in a relevant field (e.g., social work, nursing)

What We Offer:
- Competitive salary of £25,000 - £32,000 per annum
