Care and Support Coordinator

We are seeking a dedicated and caring individual to provide person-centred care and support in our Leeds Extra Care Service, enabling older people to enjoy an active and independent lifestyle.
As a practitioner, you will be responsible for delivering warm, flexible and experienced care and support to individuals with diverse care needs. You must be enthusiastic about the provision of services and have a good insight into current thinking and practice in the field.
You will need skills in positive communication and engagement, as well as the ability to demonstrate unconditional positive regard for the people we support. Needs assessment and proactive risk management are essential skills for this role.
