About the Role:

We are looking for a skilled Care Coordinator Professional to join our team at Entrust Social Care Ltd. As a Care Coordinator, you will be responsible for coordinating care services for individuals with complex mental health needs, working closely with healthcare professionals and service users to develop and implement personalized care plans.

The ideal candidate will have a strong background in social work, with a minimum of 2 years' experience working in an adult social work setting. They will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. The successful candidate will also have a deep understanding of statutory responsibilities, including the Care Act and safeguarding vulnerable adults and children.

Key Responsibilities:
  • Coordinate care services for individuals with complex mental health needs
  • Work collaboratively with healthcare professionals and service users to develop and implement personalized care plans
  • Monitor and review care plans regularly, making adjustments as needed to ensure optimal outcomes
  • Liaise with other professionals, including GPs, hospital staff, and community agencies, to ensure seamless care coordination

Requirements:
  • DipSW or equivalent qualification
  • Registration with the HCPC
  • CURRENT DBS

Salary Range: £40,000 - £45,000 per annum
