About the Role

We are seeking a skilled Care Coordinator to join our team at Cygnet Oxley, where you will play a vital role in delivering exceptional care to adults with autism and learning disabilities.

Job Description:
  • You will work closely with a clinical team to develop and implement personalized support plans that meet the unique needs of each service user.
  • Your responsibilities will include providing guidance and emotional support, adapting help to individuals' specific needs, and reporting on medical and welfare needs as required.
  • You will also be responsible for maintaining a safe and clean environment, ensuring the well-being and safety of all service users.
Requirements:
  • To be successful in this role, you will need to have a FULL UK driving license for a minimum of 2 years and be confident in your ability to deliver exceptional care in a specialist enhanced support environment.
  • You must be an excellent communicator and team player, sensitive and intuitive, and passionate about empowering and supporting service user independence.
  • Prior experience working with adults with autism and learning disabilities is essential, as is a genuine drive to make a real difference in the lives of those you support.
