Job Title: Adults Care Manager

We are seeking an experienced Qualified Social Worker to join our Adult Community Mental Health Team as a Care Manager. This is a rewarding role that will allow you to work closely with vulnerable adults, providing assessment, care planning, and review services.

As a Care Manager, you will be responsible for a caseload of service users with complex mental health needs. You will assess their needs, develop and implement care plans, and review and assess these regularly. Your experience working with people with mental health needs, care planning, and risk assessments is essential for this role. Additionally, you will need to have knowledge of statutory responsibilities, such as the Care Act and safeguarding vulnerable adults and children. You will also be required to liaise with other professionals to ensure comprehensive support for your clients. The successful candidate will need to hold a DipSW or equivalent, must be registered with the HCPC, have a current DBS, and be a car driver. Estimated salary: £38,000-£42,000 per annum.
