Job Title: Patient Care Coordinator Assistant

About the Role:

Digitaltidewave is seeking a highly motivated and compassionate Patient Care Coordinator Assistant to join our healthcare team. As a key member of our team, you will play a crucial role in coordinating patient care, supporting Registered Nursing Staff, and maintaining patient dignity and privacy.

Main Responsibilities:

  1. Assist in implementing care plans delegated by Registered Nursing Staff, ensuring seamless delivery of patient care.
  2. Support patients with daily living activities, including personal care, hygiene, and sanitary needs.
  3. Contribute to maintaining accurate and up-to-date patient records, ensuring effective communication among healthcare staff.
  4. Collaborate with healthcare professionals to identify and address skin and pressure area care needs, preventing pressure ulcers.
  5. Plan and participate in therapeutic recreational activities for patients, promoting physical and emotional well-being.
  6. Work collaboratively with the healthcare team to assess, implement, and evaluate patient care plans.
  7. Develop and maintain effective relationships with patients, families, and healthcare staff, ensuring open communication and empathy.
  8. Ensure a clean and safe environment for patients, adhering to infection control guidelines.
  9. Monitor and report any changes or concerns in patient condition to the responsible nurse.
  10. Support registered staff in taking and recording vital signs, contributing to comprehensive patient assessments.
  11. Implement measures to protect individuals from abuse and report any suspicions.

Requirements:

This role requires a strong commitment to delivering exceptional patient care. The ideal candidate will possess excellent communication skills, be able to work effectively in a team environment, and demonstrate a passion for maintaining patient confidentiality.

The salary for this position is approximately £22,000 - £25,000 per annum, dependent on experience.
