Complex Care Coordinator

NHS West Yorkshire Integrated Care Board is seeking a Complex Care Coordinator to lead the development and management of a coordinated approach to NHS Continuing Healthcare (CHC) and Funded Nursing Care (FNC). This role requires expertise in managing complex care cases, ensuring they meet patient needs and provide value for money.
The successful candidate will have specialist knowledge of complex care needs and experience in multidisciplinary team working. They should also possess strong leadership skills, with the ability to manage and supervise staff effectively. Additionally, they should be able to assess and review individuals potentially eligible for CHC or FNC.
