Join Our Team as a Social Worker

We are seeking an experienced Social Worker to join our team at Connect2Kent, a leading provider of social services in West Kent. As a Social Worker, you will play a vital role in supporting disabled children and their families, working collaboratively with multi-disciplinary teams to deliver effective interventions and make a meaningful difference in the lives of our clients.

Key Responsibilities:
  • Conduct comprehensive assessments of children with disabilities and their families, identifying their needs and developing personalized care plans.
  • Work closely with healthcare providers, educators, and community organizations to ensure seamless support and coordination.
  • Provide direct support, advocacy, and guidance to children and families, empowering them to achieve their full potential.
  • Maintain accurate and detailed case records, adhering to legal and organizational standards.
  • Offer mentorship and support to newly qualified Social Workers and other team members, promoting professional growth and development.
  • Engage in regular supervision and ongoing professional development to stay current with best practices and policy changes.
