Job Summary

Main Responsibilities
  • Conduct comprehensive assessments and develop care plans in collaboration with service users and their carers.
  • Evaluate treatment plans and deliver safe and effective home treatment for identified client groups.
  • Triage and assess referrals received by the service, working in partnership with multi-disciplinary colleagues.
  • Coordinate the work of the team, ensuring seamless delivery of services.

  • Be a Registered Mental Health Nurse as defined by the Nursing and Midwifery Council and be active on their register.
