Job Summary

The Midlands Partnership NHS Foundation Trust is seeking a skilled Community Mental Health Nurse to join our team in Telford. As a key member of our Urgent Care Pathway, you will work 24/7 to provide mental health assessment, treatment, and support to individuals in Telford & Wrekin.

Main Duties

As a Community Mental Health Nurse, you will:

  • Provide specialist assessment and care planning in collaboration with service users and carers
  • Evaluate treatment plans and deliver safe and effective home treatment
  • Triage and assess referrals received by the service, working closely with multi-disciplinary colleagues
  • Coordinate the work of the team
