About the Role

We are seeking a skilled Community Mental Health Nurse to join our team in Telford. As a key member of our Crisis Response Team, you will provide specialist assessment and care planning in conjunction with service users and carers.

Key Responsibilities

  • Provide specialist assessment and care planning in conjunction with service users and carers
  • Evaluate treatment plans and undertake home treatment
  • Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues
  • Co-ordinate the work of the Team

Person Specification

  • Be a Registered Mental-health Nurse as defined by the Nursing and Midwifery Council and be active on their register
  • Previous experience of working in the community
