Senior Community Mental Health Nurse Specialist

An exciting opportunity has arisen for an experienced Senior Community Mental Health Nurse to join the Assessment and Diagnosis team within the Dementia Assessment and Support Service (DASS).
We are in an exciting period of change due to the Dementia Transformation taking place across Shropshire, Telford & Wrekin.
This role involves working closely with our Nurse Practitioners who assess and diagnose Dementia, to support the referrals management process and to provide community-based intensive input to our service users and carers who have more complex needs, whilst in the assessment process, and this will be driven by clinical need.
Main Duties and Responsibilities
- Referrals Management: including triage, risk assessment and risk management, care planning, MDT working, and liaison with external services.
- Care Planning: developing, implementing, and evaluating programmes of care.
- Medication Administration: complying with relevant Trust procedures and NMC Guidance.
- Therapeutic Interventions: delivering a range of interventions in accordance with the agreed plan of care and Pathway guidance.
- Risk Management: completing and contributing to robust risk assessments and risk management plans.

Key Requirements
- Taking responsibility for the management of a complex caseload, including specialist assessment, care planning, and evaluation of individual treatment plans.
- Participating in the setting of quality standards, including auditing, monitoring, and reviewing practice in line with current clinical guidance, practice, and policy.
- Assessing patient's individual care needs, developing, implementing, and evaluating programmes of care.
- Delivering a range of interventions in accordance with the agreed plan of care and Pathway guidance.
- Completing and contributing to robust risk assessments and risk management plans.
